Patients Home City & Incident City "not a valid value of atomic type 'CityGnisCode'

When you get this error, it's just a mapping issue. The process is the same for BOTH errors. 

1. Click to the right of address, and select Google Address selection
2. click next to the CITY, and select city from City Search
3. Done!
